Private/Closed adoption, attorney was Donald R. Siegel, Esq. Birth Mother's name at time of birth was Paula Cook. Hospital of birth was North Shore University Hospital in Manhasset, NY. Delivering Doctor was Erwin O. Kubec, MD [deceased]. Adopted in Mineola, NY at Nassau County Surrogate's Court on 08/14/1984 (date of decree). I also have plenty of Non-ID info about my birth mother. If anyone at all can help me find my birth mother and/or birth family, it would be very well appreciated.
